Hospital Errors Occur More Frequently Than Reported

A recent study published in Health Affairs Journal found that the adverse event detection methods used to track patient safety in the United States today failed to detect 90% of the adverse events occurring in hospitals.  The study examined three teaching hospitals in the United States and found that voluntary reporting system under reported the number of adverse events. The study concluded that an adverse event occurred in one-third of all hospital admissions. Bloomberg News
